B. Transferees

1. Certificate of transfer credentials.
2. Informative copy of subjects taken with grades and credits earned in college/university last attended.
3. Certification of good moral character duly signed by Dean / Guidance Counselor / Student Affairs Officer of last school attended.

C. Old Students

1. Must have complete grades (previous school year).

1. Family must be included in the DSWD’s Listahanan 2.0. Please bring your DSWD number.
2. Report Card (Form 138-A) indicating promotion to college signed by the Senior High School Principal.
3. Certification of good moral character duly signed by the Senior High School Principal or Guidance Counselor.
